  To be used by Club Leaders to nominate their members for Medal & Blue Awards and also to nominate them for Outstanding Jr and Outstanding Sr Member.

  To be used by the leader of the 4-H club to submit their county and state 4-H project members.
  
  
• Scoresheet for 4-H Honor Clubs
  To be used by Club Leaders and submitted by October 1st.
  
  
• Standards of Excellence for Clubs
  To be used by Club Leaders and submitted to Extension Office by October 1st.
