  Must be filled out by prospective volunteers and turned into the Extension Office.
  
  
• Extension Volunteer Application
  All prospective Extension volunteers must fill out a volunteer application and return it to the Extension office.
  
  
• Extension Volunteer Behavior Guidelines
  
  
• Extension Volunteer Cover Letter
  
  
• Illinois Police Conviction Check
  Must be filled out and turned in order to volunteer for youth related activities at Extension
