Chronic Disease Self Management Program Available
The "Live Well, Be Well" program is a Chronic Disease Self Management Program that is open to all who have a chronic long term condition such as arthritis, heart disease, diabetes, osteoporosis (and many others) or are a caregiver of someone with these conditions. The program consists of 2 hour, once a week sessions, for six weeks, in a friendly and comfortable environment. Participants will learn to manage their symptoms by learning to:
- better communicate with doctors, dieticians and nurses
- reduce frustration
- fight pain and fatigue
- make daily tasks easier
- reduce stress and anxiety
- make informed treatment decisions
- manage medications
- break the symptom cycle - take back control of your condition
Participants will learn a healthy way to live with their chronic conditions.
The Extension office would like to know who would be interested in taking this workshop, and then will try to schedule the sessions at everyone's convenience.
If interested, please contact the Extension office at 543-3755 or e-mail Cheri Burcham (cburcham@illinois.edu) with your name and contact information. This is a very valuable program and it would be great to get it off the ground!
