Financial Statement of Plans All clubs who maintain a club treasury must fill this form out annually as well as have their treasurer's book audited.
FORM - Club Basics This form provides us with when your club meets and what adults are volunteering in your club.
FORM - Club Goals Part of the Club Award of Excellence requires clubs to set annual goals. Complete this form once your club has set goals. Return them to the Extension Office, so we can file them in your folder.
Form - Club Program Schedule Mini-Calendar that helps you keep track of month-to-month activities and what committees will be meeting.
FORM - Club Reporter Form Generic form for club reporters to fill and and submit to the newspaper and the Extension Office.
FORM - Group Enrollment Form Leaders must turn in this form if a club would like to do a project together as a group.
FORM - Organizational Leader Enrollment Your clubs organization leader should complete this form and turn it in to the Extension Office. The organizational leader receives all correspondence from the office.
