The links to files below for the "Standards of Excellence & Achievement and the My 4-H Project Record forms" can be valuable to 4-Hers, not only in the year that a project is studied and exhibited, but also in the future as members look back at what they have accomplished and learned in 4-H.

Key facts about the Standards of Excellence & Achievement and the My 4-H Project Record form:

  • All members receive a Standards of Excellence & Achievement form with their project books at the time of enrollment.
  • Depending on the projects enrolled in, members will get ONE copy of the My 4-H Project Record form in EACH Award/Curriculum area.
    • Example: If you enroll in horses, cats, clothing you will receive an Animal Science and a Home and Family form.
  • These forms are available online for members to fill out and carry on from year to year. You do not have to use a new sheet or form each year if you are typing them on a computer. Just be sure to put the year down with new information.
  • If several projects are taken spanning several curriculum areas members can CHOOSE to focus on a few Award/Curriculum areas so that they do not have to fill out a My 4-H Project Record form in each area.
    • Example: If you are enrolled in visual arts, leadership, breads and cereals, food demonstration, beef, horses, horticulture and welding: You can CHOOSE to focus on Personal Development, Food, Nutrition and Health and Animal Science.

The goal and or intention of the forms are to:

  1. Give 4-H members a written/typed record of their experiences in 4-H.
  2. Remind members of the Standards for Excellence and to encourage participation in all that 4-H offers.
  3. Increase member participation in completion of the Illinois 4-H Award Application to receive advanced awards and recognition in 4-H.
  4. Provide an opportunity for members to think and express their ideas and prepare for future resume writing and career prospects.

4-H members need to fill out the Standards of Excellence & Achievement Sheet and a minimum of one My 4-H Project Record forms OR fill out the Illinois 4-H Award Application.

The record forms below are recommended for 4-Hers of all ages. However, if the 4-Her is filling out the Illinois 4-H Award Application for county/state awards, filling out the following forms are not necessary.
