Instructions to complete Registration Sheets
INSTRUCTIONS FOR COMPLETING 4-H SHOW
REGISTRATION SHEETS
Please read and follow all instructions before you start filling out premium sheets.
General Information
You will be using premium sheets (called MARSHALL-PUTNAM 4-H SHOW REGISTRATION) which will become a legal document for your entry and payment of premiums. One premium sheet in triplicate is available. The pink & yellow copies are for the exhibitor and their 4-H leader. The white copy goes to the Extension Office.
Instructions
1. Premium sheets are printed on NCR (No Carbon Required) paper. Any mark or scratch will print through to the second and third sheets. PLEASE BE CAREFUL, THIS PAPER WILL NOT ERASE.
2. Use a ball point pen or typewriter on all premium sheets. PRINT FIRMLY - DO NOT WRITE.
3. Print or type your name, 4-H age, your club, your parent(s) name and phone number.
4. In column headed Dept., place the letter or letters of the department. (B, SH, G, D, SW, HP, P, R, DC, MSE, A, NRE, H, V, C, HC, CT, F, Mc).
5. In the next column, enter the Class number (#).
6. For appropriate livestock entries, enter the breed name in the last column.
7. Sign and date the form.
8. Members must be enrolled by the appropriate project deadlines (January 1/May 1) to be eligible to register for the 4-H Show.
9. All exhibits must be registered by the July 1 deadline on the 4-H Show Registration form to be considered for awards, premiums and State Fair.
10. Registration forms are due in the Extension Office, 509 Front Street, Suite 4-H, Henry, IL 61537, by July 1.
