• 4-H Fundraising Approval Form
  Illinois 4-H clubs should receive advance approval from the Macoupin County Extension unit staff using the 4-H Fundraising Approval Form when planning or conducting a fundraiser.

  This form must be completed for medium/high risk activities where 4-H youth will be award from the local area and away from parents especially overnight.

  All 4-H volunteers are required to complete a CANTS Authorization Form. Please submit the authorization to the Extension office.

  This fact sheet highlights the guidelines for club fiscal policy including checking & savings accounts, fund raising activities, charitable activities, reports and audits and sales tax exemption.
  
  
• Conviction Information Name Check Request Form
  All 4-H leader volunteers are required to complete a Conviction Information Name Check Request Form. Please submit it to the Extension office.

  4-H Club are prohibited from signing any contracts. The following contract has been approved by the campus legal department to use when 4-H club wish to use the Lake Williamson Christian Center facilities.
