Volunteer Information

If you would like to become a volunteer for youth programs in Kankakee County, please read the Volunteer Application Cover Letter First and then fill out all forms. If you have any questions, feel free to contact Extension at 815/933-8337.

• Child Abuse and Neglect Tracking System (CANTS) Authorization for Background Check Form
  This form is used to perform a background check through the Illinois Department of Children and Family Services.
  
  
• Conviction Information Name Check Form
  This form is used to perform a criminal conviction check through the Illinois State Police.
  
  
• Extension Volunteer Application Form (Form 90109)
  Volunteer Application Form needs to be filled out completely (including references). Letters will be mailed out to your references, so please be sure to contact them and see if it is alright to use them as a reference.
  
  
• Volunteer Application Cover Letter
  Please read this letter first. Then fill out all accompanying forms and return them to the Extension Office.
