Award Information

• 4-H Alumni Award Application Form
  The Alumni Award provides recognition to 4-H alumni whose contributions to society exemplify effective citizenship, leadership, and public service success in their chosen careers and a sustaining interest in 4-H and similar worthy community endeavors. This form must be turned in to the Extension Office no later than October 1.
  
  
• Friend of 4-H Award Nomination Form
  The Friend of 4-H Award is presented to non-4-H individual or group who has contributed significantly to support of 4-H programs. This form must be turned in to the Extension Office no later than October 1.
  
  
• I Dare You Award Nomination Form
  The I Dare You Award winner should exhibit development in leadership, scholarship, and character not only in 4-H but also, church, school, and community. Form must be turned in to the Extension Office no later than October 1.
  
  
• Kankakee County 4-H Awards Application Form
  This award application should be turned in with all other records by the designated date in August.
  
  
• Key Award Nomination Form
  The Key Award provides recogntion to older 4-H'er who must meet Leadership Standard, listed in Clover, for 2 years. This form must be turned in to the Extension Office no later than October 1.
  
  
• Listing of Kankakee County Awards Available to Turn in
  This gives you a complete list of all awards available and dates they are due. It also lists all records forms which must be turned in to receive awards.
