• Child Abuse and Neglect Tracking System (CANTS) Authorization for Background Check Form
  This form is used to perform a background check through the Illinois Department of Children and Family Services.
  
  
• Conviction Information Name Check Form
  This form is used to perform a criminal conviction check through the Illinois State Police.
  
  
• Extension Volunteer Application Form (Form 90109)
  Volunteer Application Form needs to be filled out completely (Including references). Letters will be mailed out to your references, so please be sure to contact them and see if it is alright to use them as a reference.
  
  
• Kankakee County 4-H Superintendent and Assistant Superintendent Application Form
  If you are interested in becoming a Superintendent or Assistant Superintendent, please fill out this form. By filling out the form, it does not mean that you will automatically be appointed. Several things must happen. 1. There has to be an opening in the department. 2. The application is referred to the appropriate committee (livestock or general projects) for approval. Upon approval from this committee it goes to the Youth Program Committee for final approval. 3. You must have current volunteer application forms on file.
