Enrollment

In this section, you will find all of the paperwork related to enrolling in 4-H clubs and/or projects.

• Clover Challenge Agreement Form
  This form is for youth ages 15-18 to continue exploring a state 4-H project area after successfully completing the other levels of a project area. These projects MUST relate to a current 4-H project. Plans MUST be submitted to the Extension Office for approval. An adult mentor should assist youth with this in-depth study.

• Individual Enrollment Form
  This form must be completed for all members and adult volunteers.
  
  
• Project Selection Form 2009-10
  This form allows current 4-H members to add or delete projects.
