Foundation Scholarships
Qualifications
A. Must have completed at least one quarter or semester of study at a university, college, junior college, trade or vocational school. A copy of your most recent college transcript must be attached or your application will not by reviewed.
B. Must have completed three or more years of 4-H work in Cook County.
C. Must have need for financial assistance for undergraduate study only.
D. You may receive this scholarship only once.
E. You may apply for this scholarship more than once.

What is the value of the scholarship?
The donor determines this. In recent years scholarships have been $500.
How many scholarships are given annually?
The number of donors providing funding for the scholarships determines the number of scholarships that are awarded.
How do I apply for the scholarship?
To apply, secure a scholarship application from your unit office or the Chicago/Cook County 4-H Foundation, Illini Center, 200 S. Wacker Drive, Chicago, IL 60606. Telephone 312-575-7840. Applications must be submitted by the deadline established by the Foundation. The deadline is September 1 of the current year.
How often are scholarships awarded?
Scholarships are awarded annually. The recipients will be notified of who the donor is along with the address of the donor.
What are the responsibilities of the recipient(s)?
Recipients are expected to send a thank you letter to the donors and the Chicago/Cook County 4-H Foundation at the time they receive the scholarships. An occasional note or letter to inform the donors and Foundation of their progress would be useful.

Palos Heights Youth Awarded Scholarship from 4-H Foundation
Mark Kosinski of Palos Heights, Illinois is the recipient of the 2006 Chicago/Cook County 4-H Foundation Scholarship. Mark is currently enrolled in Illinois State University, where he is pursuing a degree in Telecommunications and Business. He graduated from Lockport Township High School.
The $500 award comes from donations from a variety of businesses and individuals. It has been bestowed on many young people over the years. The current president of the Chicago/Cook County 4-H Foundation, Deidre Baumann, received the award 15 years ago. "The Foundation is pleased to offer this scholarship to Mark as a token of appreciation for his years of dedication as a 4-H Club Member. He has upheld the principles that the 4-H program seeks to instill in its participants, those of volunteerism, community service, academic strength and leadership," she says.
Ms. Baumann will be presenting the award to Mark at the annual Awards and Recognition Banquet. The banquet will take place this Thursday, November 2, 2006 at the Lake Katherine Nature Preserve, 7402 Lake Katherine Drive in Palos Heights, Illinois at 6:00p.m.
